چکیده

Puroose: Proliferative diabetic retinopathy is assumed to develop because vasogenic factors are released from retinal areas that are ischaemic and hypoxic secondary to occlusion of the retinal vascular bed. It has been propclsed that vascular occlusion in diabetic retinopethy may be due to the deposition of periodic acid Schiff positive glycoprotein compounds in the retinal vascular walls. but the role of these glycoproteins for retinal vascular occlusion has not been established. Methods; Periodic acid Schiff staining and immunohistochemistry to laminin, fibronectin, vitronectin .and type VI collagen was studied in eleven areas of capillary closure and eleven adjacent control areas from seven eyes of five diabetic patients, and in five midperipheral areas from five eyes of five age-matched controls. In each eye from diabetic patients the localized vascular lesions were identified from casts of the retinal vascular system. Beg&& In the retina from diabetic patients there was a significantly higher number of vessels showing periodic acid Schiff staining and immunoreactivity to the studied glycoproteins. However, there was no difference between the number of glycoprotein containing vessels in areas of vascular occlusion and in adjacent control areas in diabetic patients, and the material occluding the lumen centrally in areas of vascular occlusion did not show any glycoprotein staining or immunoreactivity. Conclusion% The findings confirm that glycoproteins are deposited in the vascular walls of diabetic retinopathy, but the findings also suggest that other factors are involved in causing vascular occlusion in this disease.

Cardiovascular disease in insulin dependent diabetes mellitus: similar rates but different risk factors in the US compared with Europe.

BACKGROUND Cardiovascular disease (CVD) in insulin dependent diabetes mellitus (IDDM) has been linked to renal disease. However, little is known concerning international variation in the correlations with hyperglycaemia and standard CVD risk factors. METHODS A cross-sectional comparison was made of prevalence rates and risk factor associations in two large studies of IDDM subjects: the Pittsb...

QTc Interval Prolongation Is Independently Associated With Severe Hypoglycemic Attacks in Type 1 Diabetes From the EURODIAB IDDM Complications Study

OBJECTIVE Our aim was to assess whether severe hypoglycemic attacks were cross-sectionally associated with abnormalities of the QTc interval in type 1 diabetic patients. RESEARCH DESIGN AND METHODS The study included 3,248 type 1 diabetic patients from the EURODIAB IDDM Complications Study. Severe hypoglycemia was defined as an attack serious enough to require the help of another person. A co...

Serum total renin, an independent marker of the activity and severity of retinopathy in patients with IDDM.

BACKGROUND/AIMS Recent studies have demonstrated marked renin and prorenin concentration gradients between ocular tissues and blood, and local expression of the renin-angiotensin system (RAS) in the eye. The authors determined whether serum total renin, which mostly consists of prorenin, is a marker of the activity and severity of diabetic retinopathy independent of other microvascular complica...

Race and Ethnicity

IDDM is one of the most common chronic disorders of childhood (see Chapter 3). Standardized descriptive studies of the epidemiology of IDDM are being conducted around the world, providing much needed information regarding the frequency and potential risk factors for IDDM in developed and developing countries. In many areas, these investigations are being facilitated through the World Health Org...

Free and total insulin-like growth factor I (IGF-I), IGF-binding protein-1 (IGFBP-1), and IGFBP-3 and their relationships to the presence of diabetic retinopathy and glomerular hyperfiltration in insulin-dependent diabetes mellitus.

The existing literature on serum insulin-like growth factor I (IGF-I) levels in insulin-dependent diabetes mellitus (IDDM) is conflicting. Free IGF-I may have greater physiological and clinical relevance than total IGF-I. Recently, a validated method has been developed to measure free IGF-I levels in the circulation. Serum free and total IGF-I, IGF-binding protein-1 (IGFBP-1), and IGFBP-3 level...
